STRONGER PARTNERSHIPS

As well as supporting NDIS participants through the SDA, we also work with a wide range of professionals who are working with participants to achieve their goals – including support coordinators, occupational therapists, local coordinators, and allied health professionals.

We aim to build strong partnerships with professionals by working together to ensure participants are receiving the best quality care and support. If there is an area where we cannot provide support, we will endeavour to seek avenues of support to meet the needs of professionals and participants.

Who is eligible for Ozability Homes accommodation?

Ozability Homes is a trusted accommodation option and housing solution for participants who are:

• Ready to leave hospital but waiting for a specialist disability accommodation vacancy or build to be completed.
• NDIS Participants in Residential Aged Care wishing to leave
• Are seeking transitional accommodation between SDA & other accommodation options
• Are seeking for respite and/or short term accommodation to give themselves or their carer, guardian and/or parents a break

REFERRAL PROCESS

IN 5 SIMPLE STEPS

Complete our referral form and a Ozabilty Homes Specialist will contact you to discuss requirements and eligibility.

We will identify properties fulfilling referral criteria (includ. participant +/- support requirements, location, dates and budget). Quotation will then be sent to referrer.

Referrer confirms NDIS approval to proceed and we will arrange a Service Agreement.

Ozability Homes Service Agreement Signed & returned to secure housing. We will liaise with referrer & team to coordinate equipment deliveries & access.
5
Final confirmation for seamless transition and particpant arrival. Ongoing feedback and monitoring to assist particpants and supports.
